User Authorization Problem Report Form

Introduction

Before you complete the Problem Report Form, you may want to read the topics below. When you click on one of the links below, your browser will open a new window. When you finish reading the information, close the browser window to return to this form.

This form is for users of PC Access for Windows who have received a User Authorization error. Before you can fill out this form, you need to have run the User Authorization manage to generate your Serial Number and Validation Code. For instructions on how to do this, see: Using an Install Count or Requesting a Reset Code.

Correcting the User Authorization Error (required)

If you do not have your Serial Number and Validation Code, please follow the instructions here: Using an Install Count or Requesting a Reset Code. MFM Technical Support will need your Serial Number and Validation Code to correct your User Authorization Error.

  • Note: Your Validation Code is not your four digit Registration Code.

  • Note: You will need your original PC Access for Windows Disk 1 to generate a Validation Code, Serial Number and to reset your User Authorization.
